Diagnostic imaging: (e.g., MRI scans, CT Scan, Ultrasound)- the actual films or CD-Rom; and a copy of the reports
Blood tests reports
Prior consultations: Operation reports, Radiation therapy reports, Chemotherapy reports
Biopsy reports, Pathology reports, Pathology slides (if possible)
